چکیده

Aminoglycosides are an important group of antibiotics whose basic compounds contain aminosugars. They are produced either by Streptomyces spp (streptomycin, kanamycin, tobramycin, and neomycin) or Micromonospora spp (the gentamicins and sissomicin), which accounts for the different spellings. Semisynthetic derivatives have also been produced (for example, amikacin from kanamycin and netilmicin from sissomicin), and further such compounds are under investigation. Aminoglycosides are not appreciably absorbed from the gut in normal subjects and must be administered by intravenous, intramuscular (or subcutaneous) injection to treat systemic infections, although in patients with impaired renal function prolonged oral administration (as, for example, neomycin in liver failure) may lead to toxic accumulation of the drug in the body. Aminoglycosides are well absorbed from injection sites usually, from the peritoneal space, or from large wounds and granulating areas if applied topically. They are distributed throughout the extracellular water compartment of the bodyachieving concentrations quarter to half of those attained in the serum, in pleural, ascitic, and joint fluids, in bronchial secretions, and in bile. But they pass poorly into the cerebrospinal fluid even when the meninges are inflamed. The aminoglycosides are excreted by glomerular filtration in an active form. Their antimicrobial effect is enhanced by an alkaline pH. When renal function is impaired, serum and tissue concentrations may readily increase to toxic levels. This necessitates regulating dosage carefully in such patients either by serum assay or nomogram based on the serum creatinine concentration.

Resistance to Aminoglycoside Antibiotics in Escherichia coli Isolated from Urinary Tract Infection in Iran in 2000-2012: A Systematic Review

Background and Objectives: Resistance to aminoglycosides has become a major problem in the treatment of urinary tract infection due to the extensive use of this antibiotic group. Due to the increased resistance of Escherichia coli to aminoglycosides, this study systematically evaluated the resistance to aminoglycosides by Escherichia coli isolated from urinary tract infection (UTI) in Ira...

Survey of Aminoglycoside Acetyl Transferase Genes in Multi-Drug Resistance Acinetobacter

Acinetobacter baumannii is a gram-negative, non fermenting cocobacills that its species are opportunistisic pathogens and cause nosocomial infections among patients in intensive care unit (ICUs).This bacteria cause various infections such as pneumonia, meningitis, endocarditis, and urinary tract [1]. Three decades ago, Acinetobacter baumannii infections treated with traditional antibiotics but ...

Prospects for Success in Inhibiting the Growth of Multidrug-resistant Bacteria with Paromomycin

Streptomyces is the most important bacterial genus known to produce secondary antibiotic compounds. Considering the speed of bacterial resistance spread, it becomes more important to study less used compounds. Aminoglycosides disrupt the function of the target microorganism by acting on the protein-synthesis process. Paromomycin is an aminoglycoside that acts on a wide range of gram-positive, g...

Optimal Aminoglycoside Therapy Following the Sepsis: How Much Is Too Much?

Severe sepsis and septic shock are major problems as the result of high rates morbidity andmortality in intensive care units (ICUs). In the presence of septic shock, each hour of delay inthe administration of effective antibiotics is associated with a measurable increase in mortality.Aminoglycosides are effective broad-spectrum antibiotics that are commonly used in ICUs forthe treatment of life...
